Welcome to Thoracic and Cardiovascular Institute, PC TCI is mid-Michigan's comprehensive healthcare delivery system providing prevention, diagnosis, treatment, rehabilitation, and education to patients with cardiovascular, thoracic, and vascular conditions.
Above everything else, Thoracic and Cardiovascular Institute puts patient care first.

Appointments
You can call to make an appointment directly or by physician referral. When scheduling your first appointment with TCI, or if it has been two years or more since one of our physicians has treated you, you will receive an information packet that contains the following:
A letter confirming the date, time, and location of your appointment including a map
Patient Information Form
Health History Questionnaire Form
Financial Policy, with a list of Participating Insurance Carriers
Find Your Appointment
Please bring with you to your appointment:
Completed Patient Information Form (previously sent to you)
Completed Health History Questionnaire (previously sent to you)
Your insurance card(s)
Pre-authorization form(s) if your insurance company requires them for specialty care, unless they were mailed by your primary care physician
Your current medication bottles
Diagnostic reports and/or x-rays (as requested by TCI scheduling staff)
A written list of question/concerns that you would like to discuss with your physician
Disability forms if applicable
When you arrive, please check in with a receptionist. The receptionist will request the information that you have provided for your visit. The completed forms will be attached to your medical record.
Unforeseen urgent or extended medical care can sometimes interfere with appointment times. Your courtesy is greatly appreciated when your appointment has been delayed. We will make every effort to make your waiting time as comfortable and timely as possible.
If you are unable to keep an appointment, please be sure to cancel it so it can be given to another patient.
Diagnosis
In the event that non-invasive diagnostic tests are indicated to aid in the assessment of your evaluation, a nurse or technician will explain and perform each procedure. Once your assessment has been completed your physician will explain the test results to you and any suggested treatment. Please do not hesitate to ask questions concerning your condition and/or suggested treatment - understanding cardiac, thoracic, and vascular disease is the first step in the treatment program.
Treatment
Medical technology continues to provide improved methods for treating cardiovascular disease. If your suggested treatment indicates that hospitalization is required for out-patient (not admitted) or in-patient (admitted) procedure, arrangements will be made for your admission. Please inform the scheduler of your admission. Please inform the scheduler if accommodations for out-of-town family and friends are needed during your stay in the hospital in order for them to be close to the hospital.
Urgent Problems/Emergencies
For urgent problems call TCI. A physician or nurse is available 24 hours a day to assist you. If you require emergency care, go to the nearest room or call 911. The emergency room staff will contact your doctor.
Your Primary Care Physician
Your primary care physician is an important member of our healthcare team. While you are our patient, we will provide written and verbal reports to your family physician on your progress. Please be prepared to provide the name and mailing address of your physician(s).
